shall not be held responsible or legally liable for any losses of personal property or for
any bodily injuries, or results thereof, incurred and suffered by the applicant on any
property of Shiloh Inc. or in connection with any activities or programs, unless such loss
or injury results directly from the negligent or willful act of the staff not acting within
the scope of their responsibilities.”
Health Form: Once your registration form is received we will mail you a
health/ medical release form that will need to be filled out and brought back on
the first day your drop off your child. (Please note your child will not be able to
participate in the free day camp without completed form.)
Please check the activities that you give parental consent for your child to
participate in:
1. "I am the Parent or Guardian of the camper listed on this form and have read the general consent and release"
2. "I Understand that by submitting this form it does not guaranty a spot for my child, due to limited space" (You will receive a email verifying available space)

Free Day Camp Registration
Who can attend?
Any local child age 7-12 is welcome! But there is only a limited amount of
spaces available, so fill out the registration form send it in and come to the free day camp!
When?
June 22-24, 2011
8:00am to 2:30pm.
Check-in will be from 8:00-8:20am
Activities will begin at 8:30!
Pick-up will be at 2:30.
Why have a Free Day Camp?

Free Lunch!
Lunch will also be provided for all the kids! So all your child will need to bring is play clothes and a good attitude!
